"Better" in what sense? It depends on who is the patient. , I suppose you have a choice in the field. What does the surgeon recommends? Since you asked, here is my point of view of the reading I've done on the subject (who had replaced the two hips). If you are young and female, choose to ceramics. There is concern about the metal ions and women of childbearing age. If you are young and male, I think metal / metal.
