Tips on dealing with an insurance company that refuses to pay?

My dad applied for insurance coverage last year, and received a letter dated July 19th that says his insurance was “IN FORCE”. Based on that, he scheduled a doctor’s appointment for July 24th where it was discovered that he needed a hip replacement. Later, the insurance issued several other pieces of paperwork stating that the “effective date” was July 28th – thus, they say that the condition discovered on July 24th is a “previously existing condition” and is not covered by my father’s policy.

He only made the July 24th appointment because the letter received July 19th implied – to him – that the policy was “IN FORCE”. He did not receive anything prior to July 24th stating that the policy was not effective until July 28th. But, because the policy was “effective July 28th”, the company refuses to pay.

Any suggestions on how to deal with this?

Hoping you keep a copy of the letter, you can prove the date in question. This will be a HUGE help in discussing a lawsuit against the insurance company. Once you have the date on the letter and as long as the letter doesn’t indicate another date when the policy will be in “full force” then you’re set to negotiate, otherwise it’s a you said/they state scenerio and you don’t usually win those without proof. Once you have the letter, don’t be shy about faxing it to them to make your point. They will work with you then, for sure.

Sorry I can’t be more positive.

What every employer should know the questions in an online business online

Several generations grew up in the PC era, dating from 1970. Even great inventions of the 1980s, in fact, are already obsolete video players for mobile phones the size of bricks. These new generations are PC as a part of everyday life, and are completely comfortable with very advanced video games, smart phones, netbooks, WiFi satellite radio – and, above all Internet. virtual constant connection to everything you need is a way of life.

It is almost magical, the word "Internet" and it was certainly the center of many fantasies in the late 1990s. At this point, you might get a seed funding of several million dollars by sticking a "dot com" suffix at the end of anything – Sports.com, Poker.com, Ideas.com, whatever – and recalling the game of "old" bankers and businessmen that the World Wide Web is a new type of economy. Profits are not the most important and apparently, simply appear as a result the hip is smooth, fresh and relevant.

strikes reality

The point com bust "of 2000-2001 is lying fantasies, of course, as the hype boldness, freshness and relevance can keep doors open and food on the table. The benefits do not. However, even with the high gloss, well informed of the technology bubble in stock, a certain cachet attached even to the Internet and companies that operate there. This article, then, this covering all small businesses should know the questions to a business line online, if you plan to start a business online or purchase your online business today.

You're invited, of course, further study of the different things mentioned above, for more than a thousand words can only cover so much territory. However, it is enough room to help you edit, hit points main, and guide you in the right direction. You will find that you have common sense (which is increasingly rare, of course) and some surprising comments, too. Do the research all you have to do to understand exactly what you and your car rental company.

Places Common Business

Remember, this is not a "How to start a business network" of tips, but more a collection of warning messages, such as: "Hey, remember this problem," or "avoid confusion." You know you need a business plan, you are probably aware of the technical challenges of building and operating a website – and if you do not think and do not include the construction site, place items on these subjects in top of your playlist.

Perhaps the major issues involved in an online marketing business online, while Internet access is huge, and people often find your business online, while online – with links, banners, presentations, research results, etc – There are many people more than they can offline. Do not overlook traditional marketing forms, advertising and promotion, or seriously weaken and narrow the range significantly. In fact, some of its customers in the people who use the Internet only for certain activities, such as sending / receiving e-mail and make specific purchases.

Capture Client

Most of the clients of your online business is not going to "cruise" I just stumbled on your site, although there is a new online "community marketing", trying to find a way to make money, and, stumbling through Web World Wide Web. Very often, however, customers go directly to you, directly to your URL. This should clarify to you the type and scope of marketing do.

Some business advisers deny the notion that there are different business models out of line at the front. "Business is or not," André Vmarketing.biz Gofman writes, while noting that "implementation of certain activities no doubt vary with geographic location. Also marketing efforts, online business people can overlook the financial discipline, particularly in the field of vendors and suppliers costs.

Negotiation, and other virtual

Another important area for online business owners to focus on relationships with suppliers. The best relationships Business is personal, and the old adage – "People do not do business with companies doing business with other people" – is as true as ever in the trade. The questions online, in this case there must be contact Personal communication with suppliers and, instead of relying on email, chat windows or phone.

Just because your business is online, and "virtual" Somehow, do not you or your suppliers less real and human. And beings real human wants to shake hands, look into my eyes and get a "feel" each other. Like a lot of our communication is nonverbal with others is very important to get out and meet their suppliers, vendors, customers and clients face to face. Nothing can replace the "up close and personal" touch.

The end result is the final result

Finally – although there are many other issues that runs on its new research on this issue – raises the issue of financial information. Never mind that the IRS when you do your business, they want their share, as sales tax authorities, licensing offices, etc can be used for computerized systems, the business model paperless, but the tax administrations and bureaucracies live on paper forms, ink and envelopes. Regardless of the effectiveness of your records are computerized, one of the important issues for online businesses is online records.

By Of course, your Excel Quicken and Peachtree Accounting files are properly seated on the hard drive of your PC desktop, but the IRS wants to copy some specific things. You will need develop, produce and provide various other characters, reports and lists of local, regional, state and federal different types, depending on your industry and a host of other factors. Business owners who spend too much time cruising the universe "parallel" can not forget that there are important things to take care here in our lives in four dimensions (What is the three spatial dimensions plus time, which is never enough). You would do well to address issues out of your online business before they become real problems. Any number of errors can help the earth and keep you from following your trip Cyberspace – and really would be a problem, both online and off!

Common types of Surgical Solutions for Arthritis Pain Relief

During this time, he or she should discuss all of the any potential risks involved with the procedure as well as all of the alternative measures. However, a decision is made to continue with the surgery, the doctor will recommend a procedure that he feels is best suited for the patient’s condition and then will explain what that procedure entails. Note that sometimes before surgery, doctors prescribe blood-thinning medication and advise you to do various joint movement activities or exercises in order to increase circulation. Some benefits of going the surgery route can include the stabilization, improved alignment or replacement of a joint so that you can have greater mobility, flexibility, overall use and range-of-motion. Surgery may also be able to provide some level of pain relief where other treatment options might have failed. Be aware there are possible risks with surgery, though, like blood clots resulting. And your healthcare specialist may advise against surgery if, for example, you have existing health problems that could pose possible unwanted risks. Some of these problems could include sickness or infections that would need to be healed first, being over weight, which could cause more stress and delayed recovery, lung problems or heart disease. Common types of surgery used in the treatment of arthritis follow: Arthrodesis – This is the joining together or fusion of joint bones. Arthrodesis helps stop the progression of the disease at the point of fusion, ending the pain. Loss of the joint’s flexibility can result. However, the area will be better able to handle weight and offer general movement. In other words, the joint may not be 360-degree-flexible any longer, but there will be some lesser-degree of flexibility overall without pain. Arthroplasty – This is the replacement or rebuilding of an entire joint. And it is intended for those with high-level pain and movement impairment. It is discussed in much more detail in the section immediately following this one, offering a focus with regards to hip replacement. Arthroscopy – Growing more popular with the improved use of technology, this procedure is when specialized instruments are inserted into the joint by means of tiny incisions. During this surgery, the operating physician has helping staff members who aid the computerized monitoring and some handling of the instruments. The doctor can see the joint on closed-circuit television and make repairs to it, while talking through a microphone. Then not only are there physical results, but the patient has a video with audio record of the entire occurrence for insurance and other records. Arthroscopy is often performed on an outpatient basis. And recovery time is generally much less than with other “open” surgery. Osteotomy – This is a medical procedure that makes an actual cut in the bone, most generally to correct youth deformities like in the knee or hip. Osteotomy helps with re-alignment, stabilization, pain relief and the delaying of joint replacement alternatives for up to 10 years. Synovectomy – This procedure, generally done via Arthroscopy, involves the removal of diseased joint tissue lining or synovium. Results can include swelling decrease, pain relief, improved but not completely healed joint health. Joint Replacement There are currently many options in orthopedic (bone) surgery for people with arthritis. Joint replacement is the most common option. According to the National Joint Replacement Foundation, (NJRF) over 435,000 Americans underwent this procedure last year. These numbers have boosted joint replacement to one of the most successful medical discoveries and the absolute most significant surgery in the field of arthritis treatment. Joint replacement is the process of removing one’s entire joint as well as any damaged tissue and replacing it with a metal prosthesis. This prosthesis provides the patient with much need relief from pain. This surgery most effective on the weight bearing joints such as the knees, hips, and ankles, however, it has been used for all joints with successful results. Hip replacement surgery consists of removing the entire hip joint and replacing it with artificial components. These components function in the same manner as the natural hip, with the same type of motion. When a patient elects to undergo hip replacement surgery, they have an option to use their own blood. There is a great loss of blood during the procedure, and patients are prepared for this ahead of time. They can elect to have their own blood taken and stored ahead of time so that when they need a transfusion, they can use blood from their own body, eliminating many of the risks associated with transfusions. This particular procedure begins with an initial incision. The surgeon will then proceed to remove the entire hip joint, including the ball, socket, and top of the femur. Once the joint, and all damaged tissue is removed, a metal cup is adhered to the pelvic bone. Then, a metal stem is inserted into the femur; leaving a portion exposed at the end for several inches. The doctors place a ball on the end of the exposed portion of the metal rod, and all of the exposed parts are lined with another antifriction material. The hip is reassembled, placing the ball joint into the socket and the incision is then closed. Fusion Arthrodesis, or bone fusion, is another optional procedure where the bones are fused together in order to prevent them from moving independently. This can be done two ways: 1. Bone Grafting is the method of stimulating fusion between two bones by placing a small piece of bone, from another region of the body, in between. This small piece of bone encourages growth for the surrounding bones, thus fusing them in place. 2. Implantation of a metal or ceramic piece, which is adhered to each of the two bones, using either screws or a special glue, thus preventing movement of the bones. Fusion is a common procedure and is used in conjunction with joint replacement surgery, which is more extreme of a procedure then bone fusion alone. During a procedure called Ostheo, doctors can evaluate the injured tissue and eliminate any loose material with the use of instruments that are inserted into the joint through little incisions in the skin. During the procedure, the surgeon can observe any damage to the joint on a closed-circuit television, and further remove any loose growths that could be the origin of pain. This sort of surgery can often be executed on an outpatient basis, and typically involves a shorter recovery stage than open/inpatient surgery. Rehabilitation times for joint replacement surgery vary from one person to the next. However, the average person has been shown to regain most functions within three weeks. A positive attitude can help to facilitate recovery. It is important for patients to participate in this by reassuring themselves as well as seeking support from support groups, family, and friends. Regardless of they type of surgery recommended, most people recommend getting at least one other opinion before proceeding. In addition, check out books, conduct your own online research, ask questions through health chat rooms, call your own local providers and learn all you can about your health condition. And if you do decide upon surgery, look and plan ahead, too. Will you need time off work? Someone to help around the house? Someone to run errands? Line up help with neighbors, friends, church members, family, co-workers and local services to pick up groceries, bring in the mail, clean house and basically keep things running in the interim. In short, take charge and reach out.

During this time, he or she should discuss all of the any potential risks involved with the procedure as well as all of the alternative measures. However, a decision is made to continue with the surgery, the doctor will recommend a procedure that he feels is best suited for the patient’s condition and then will explain what that procedure entails.

hip replacement young
I need hip replacement surgery and want to know how to find references/backround on a surgeion. what to do?

I am young to have this surgery, but I am nervous. Please someone give me good advice… thanx. oops, mean surgeon, lol!
I just have to say, you 2 gave great advice. I do want to explain, I do not want to find a dr. that does this type of surgery, I want to find out if he/she has had any formal complaints against them. I really do not think there is a way to look that up. I checked the sites, but to no avail. Unless I am doing something wrong. I, too, get CR, but not the health report. That was a great article you sent me. Thanx loads. Tweety, I will try your link again, but I did run into a brick wall.. And Kraftee, great advice, really great advice. Thanx to both of you, very, very, much, but bear with me, I want to leave the question open for just a bit longer. OK? will tell ya more later……….You gals were GREAT!! Give yourselves a pat on the back! – Lauren

Begin by looking for a surgeon who is board certified in orthopedic surgery. In addition, look for one who has done a fellowship in adult joint reconstruction and who does lots of hips in patients like you. Also pay attention to the hospital that the surgeon uses since they will be responsible for your care after the surgery. The following is summarized from an article that appears in the current issue of “Consumer Reports On Health” Feb 2009, pp.1, 4-5 entitled “Smarter hip and knee repair”.

A recent study published in the “Journal of Bone & Joint Surgery” indicates that patients are reluctant to ask probing questions when interviewing surgeons and that this can affect joint replacement outcome. Here are some Qs to ask:
How often in the past year have you performed this surgery? (more than 50 is what you’re looking for)
What kinds of joints are used and what is the surgeon’s complication rate?
Ask about the volume of hip replacements done at the hospital that the surgeon would use. High volume can mean a more experienced staff. Ask about the hospital’s infection rate. Ideally it should be 1% or less.
What are the plans for pain management?
What are the plans for rehabilitation? You want to get moving sooner rather than later.

If you are not satisfied with the reponses you receive to questions such as these, consider getting a second opinion.

Take someone else with you when you interview surgeons. They don’t have to speak but they can take notes for you. Prepare your list of Qs ahead of time.

You say you are young so at least ask about ceramic bearings instead of metal/poly bearings. If metal/metal bearing is recommended (such as a resurfacing procedure instead of total replacement) and you are a female of child-bearing age, ask about the issue of metal ions in the body. Ask about resurfacing. Although all surgeons don’t do resurfs and although the procedure is controversial, it never hurts to get their opinion. A resurf could preserve your bone and allow you to postpone the total replacement for several years.

Don’t get hung up on issues such as the size of the incision (it doesn’t matter that much and there is evidence that the complication rate may be higher with very small incisions).

FInd out the kind of anesthesia to be used. While either general or epidural/spinal anesthesia work fine, you may be allowed to choose. My surgeon didn’t care so I did my own research and made my own choice. Gave me a sense of control in a situation where I didn’t have much. Ask whether you will be donating blood before surgery to re-infuse after. I did this for one surgery but don’t think it is a universal practice these days and has drawbacks (such as making your count low even before you go under the knife). The advantage of self-donating is that you get your own blood back instead of taking blood from the blood bank. I also took blood from the blood bank for my second surgery – I was too anemic to donate for myself.

Luck to you.

Added comment: Laurel, availability of information on doctor disciplinary actions varies from state to state. If you give us the state in which your doctor practices it would help determine whether, how much and what kind of information is available to the public (online) in that state. Keep in mind that getting info on lawsuits filed does not tell you anything. Most doctors are sued eventually.

Johannesburg is an African city of note. Johannesburg is characterized by the apparent contradiction and a combination perfect irreconcilable differences. The largest city in South Africa, Johannesburg is the richest and certainly the economic power in Africa.

It is a city boom and move the emphasis is on money – whether in business or in the street – and it was from the beginning, when the world's richest gold fields were discovered in Johannesburg during the 1880s. Camera Laptop Bags: Not Just For Adults

Parents’ problems with their kids stemmed from them not spending enough time at home. Kids were always at each other’s houses or at the local watering holes. They didn’t stay at home except to sleep and eat.

Now, kids do stay at home. They don’t even leave their rooms. You’d think that this was the perfect place to punish them for their shenanigans, but these days, a kid’s room is the ultimate haven. It’s complete with their gaming consoles, PCs, DVDs, unlimited internet connection and for some lenient parents, their own phone line.

There’s nothing parents can do about it except what they always do. In any case, kids are moving out. Laptops have replaced desktops and kids just want to be on the go but still be connected. They value their ability to shout their grievances and pleasures to the world. They want people to see who they are, what they are, what they see, who they see. It’s a not a new concept but it is one that is quickly gaining momentum.

Because of this, kids and teenagers tend to carry their laptops and cameras in their school bags or keep them in their lockers. Now, here lies the problem: Security. Laptops and cameras aren’t bought with chump change. They’re expensive and hard to replace. It’s a big cause for concern to parents because they did pay for those items. For those who worked their butts off to pay for their laptops and cameras, they should know better.

A way to take responsibility for these items is to provide it with a bag that can accommodate a student’s needs. They should be able to carry it anywhere in school or outside where they can see it at all times and keep it safe. It shouldn’t be bulky because kids still need to carry their books back home. Their camera laptop bags must have the proper padding and space to house their things.

It should be able to carry chargers, batteries, memory cards and some stuff they need in class. It’s better if two or three books fit in it so they won’t have to carry to bags to school. There are companies that provide bags with cool and hip designs that kids and teenagers will love.

Most are also lightweight and compact so they won’t look like they’re going out of town. Now, you’re kids can go out with their laptops and cameras, discover the world, and come back home safe and sound with their things still intact.
