DePuy Hip Replacement Failures:

 

DePuy Orthopaedics, a subsidiary of Johnson and Johnson, is responsible for making two hip replacement devices that have been reported to have a high failure rate necessitating a second hip replacement surgery.  DePuy has issued a Hip Replacement Recall for these devices, the DePuy ASR Hip Resurfacing System and the ASR XL Acetabular Cup System.  Reports indicate that one in eight patients will be forced to undergo a second hip surgery (revision surgery) due to the ASR system defects within five years of their first implant surgery.  This Hip Replacement Recall requires that healthcare providers monitor their patients more closely.  In general hip implants last from 12 to 15 years and in some cases thirty years with good quality materials.

 

For some time, DePuy has attributed hip implant failures to surgeons not following instructions on proper implantation.  Not only has DePuy blamed surgeons for their ASR device failure but they have also blamed patients.  This manufacturer claimed that patients were careless with their new implants contributing to a further increase in failure rates.   

 

To date, there are thousands of DePuy ASR individuals that may have to have a second surgery due to this Hip Replacement Recall.  The first surgery is bad enough in terms of being painful, possible infection and rehabilitation.  A second surgery increases the potential for these side effects particularly where infection is concerned. 

 

Some orthopedic surgeons believe that the DePuy ASR cup design is the reason for a high failure rate.  It turns out that the ASR cup has a shallower design than other hip replacement systems that may be contributing to a higher failure rate.

 

Recent data indicates that the 5 year revision rate for the ASR Hip Resurfacing System runs at 12% and the ASR XL Acetabular System is around 13%.  Other data demonstrates that revision surgery was highest with ASR head sizes below 50mm in diameter and among female patients.  The major problem appears to be mechanical loosening of the implant.             

 

Acute Hip Replacement Complications for Revision Surgery:

 

A second hip replacement surgery increases the chance for dislocation of the subsequent implant along with infection, fracture and cement extrusion.  This can continue for many years and may include abnormal bone growth and bone death (bone necrosis), severe inflammation due to an immune response as well as implant fracture.

i need to know about my back and hip pain ?

well i have now been having pain in my lower back and hips for 10 years now it has been made worse from having kids now i am getting numbness in my hands and tingling in my legs numbness on my right side of my lower spine since all the numbness and tingling started about 2 years ago i had an xray of my lower spine and hips and a couple of my verabreas are fused on the right side and also fused to my hip … i am asking if that could be the cause of the pain and numbness and tingling and what could cause it ..and if not what could cause it ?

I admit it. Before the demise of my hips and their cartilage, I wasn’t quite sure what bilateral meant. I always just assumed that it had something to do with lying next to a man who wasn’t out of the closet yet or maybe a different type of lens for your bifocals. But worse than this, I didn’t care to know. Put it this way, I was more than willing to go to the grave without ever having uttered, let alone discuss the implications of the word bilateral. For the two people out there who can relate to this ignorance? Bilateral means both sides.

In the world of hip replacements, otherwise known as THRs, bilateral means you’ve had two new hips installed. Often, once one hip deteriorates, the other soon follows. It makes sense. In my case I had hip dysplasia, a word every dog lover knows. And even though I am a bit further up the food chain, my hips were shallow in their sockets and lasted about the same length as a purebred German shepherd. Before I turned 44, my athletic lifestyle had pounded away all of the catelage in my right hip. In half a year I was hobbling as I walked. In two more months I couldn’t sleep due to radiating pain. Hips are known to cause crippling pain. They also come in pairs. So, having such miraculous relief from my first hip pain made my second THR decision easy. Two years later, the hip that had always LOOKED worse on the x-rays but that had never caused me any problems (go figure. brains have a funny way of dealing with pain. WIthin six months I was back to my gimping gait and sleepless nights. I waited until my one remaining hip started keeping me awake all night and the next day I booked my surgery. Some people agonize a lot more over whether to do it or not. Once you have one THR and feel the fabulous effects, it’s easy to go bilateral, baby. Often my spinning students call me the Bionic Woman but I just smile and tell them my preferred name. I tell them that I’d rather be called the bilateral woman. I love the first line! a fu cking lot. the teeth of time. How accurately you described time and it’s haunting nature; uou speak of it as biting, how very creative.
the second line doesnt follow any rules of metrical foot. Is this an iamb? it starts out as an iamb, but doesn’t follow that. it would sound much better if you adapted a metrical foot.
the same with line 8. soft hip is nice, but eludes to many things. Use a metaphor, or else, it sounds kind of like a hip with brittle bones.
also, line 9 changes tense in a very confusing way.” Nor shining eyes reflect my ardent gaze” While you don’t think of grammer as playing a huge role in poetry, in cases such as this, it should. The soft hip “will not be felt again” must be complimented when you go on to say what shining eyes will do. “nor will shining eyes”. If you leave the will out, it sounds like “shining eyes dont reflect in my gaze” which confuses the reader.
mine right now to taste? It’s confusing; not cryptic or poetically phrased. Try “your lips are ripe; mine ready to taste” the proceeding line, fullest use of all my days, would work better with employment of imagery.
you often leave needed words out in order to sound more poetic or follow whatever flow you desired, but it often doesn’t make sense. “though now supply of days”. it needs working on.
i dont like the last line. have a go? very new age slang, not at all fitting. The poem was kicked off with such a lovely line, don’t end it with a shi tty one.

As for boiling the crab, havent you ever read any chuck palahnuik? i believe it was in survivor that his character boiled a lobster and accidently ate it before it was dead. bad idea, mister. Bad idea.

Hydroxyapatite And Titanium As Dental And Joint Implant Coatings

Hydroxyapatite (HA) and titanium are now more than ever used as coatings for joint replacement implants and dental implants. HA and titanium promote bone ingrowth between the joint implant and the patient’s bone itself. This ingrowth allows for a sturdier implant that precipitates less pain and recovery time when compared to traditional implants that contain cement.

What follows is a list of the most common types of implants for which HA and titanium are used as implant coatings and the benefits that these coatings specifically provide for each implant.

Finger: Joint replacement is recognized as a treatment option for large joints which have been severely damaged by arthritis. Most people are unaware that when small joints such as those found in fingers become painful, deformed and/or limited, relief can come by replacing the joints with titanium and hydroxyapatite coated implants. Since full function is not restored, finger implant surgery is not considered the first choice of treatment, but for those who do make the choice, the reduction of pain due to arthritis is often significant. The surgery is most valuable for patients who will regain function and who will benefit greatly from achievable pain relief.

Ankle: Total ankle replacement is an option for patients with severe arthritis. Similar to hip and knee replacement surgeries, total ankle replacement involves removing the arthritic ankle joint and replacing it with an implant. Total ankle replacement was developed in the 1970′s with limited success. The older implants often loosened or malfunctioned and frequently needed to be removed. Today, manufacturers have developed a porous titanium implant coating that has made ankle replacement surgery a more viable option. Typically, the implant consists of two titanium end plates with a refined polyethylene center. When the end plates are coated with titanium or hydroxyapatite, the coating allows the implant to mimic the motion of a natural ankle. Prior to total ankle replacement surgery, the only surgical option for patients with disabling arthritis was fusion, which completely restricts the patient’s ankle motion.

Hip Total hip joint replacement is an orthopaedic success story, enabling hundreds of thousands of people to live fuller, more active lives. Using metal alloys, high-grade plastics, and polymeric materials, orthopaedic surgeons can replace painful, dysfunctional joints with highly functional, long-lasting prostheses. Hydroxyapatite and titanium coatings allow for cement-less implants and promote bone ingrowth between implant and human bone. Today, the top performing designs all have porous titanium and hydroxyapatite surfaces that make for stronger implants and shorter recovery times.

Dental: Dental implants are available uncoated, with a titanium plasma sprayed (TPS) coating and with a hydroxyapatite coating. The choice of coating is a matter of personal preference as all coatings are successfully used by clinicians around the world. Although, hydroxyapatite (HA) coatings can help jump start the integration process in dental implants, reducing recovery time. As a result, dental implants coated with hydroxyapatite are becoming an increasingly popular choice when placing an implant.

L-Vertebrae: Degenerative Disk Disease (DDD) is one of the most common causes of lower back pain in adults. Many patients discover relief from pain caused by DDD by pursuing nonsurgical methods such as therapy, anti-inflammatory medications and weight loss. For patients who don’t experience adequate pain relief through nonsurgical treatments, lumbar fusion has long been a common surgical treatment for alleviating lower back pain. While lumbar fusion can be an effective treatment, it severely restricts motion and it does not always provide pain relief. Porous titanium coatings have allowed many medical device manufacturers to develop new, FDA approved artificial disk replacement options. Artificial disks can reduce lower back pain while also allowing the patient to retain spinal motion. Disks coated in titanium become firmly fixed in bone just weeks after the operation and typically allow for reduced pain and greater range of motion.

Elbow: Until recently, patients in need of implant surgery to relieve the pain that comes along with elbow arthritis received elbow prostheses that require cement for bone fixation. In fact, despite the increased use of hydroxyapatite and titanium as implant coatings, cement implants are still fairly common. Hydroxyapatite coatings achieve greater bone ingrowth than do cement implants. Increased bone ingrowth makes for a more durable implant that allows for increased range of motion and less pain.

Shoulder: Arthritis of the shoulder joint causes damage to bone and cartilage. If left untreated, the damage can cause a great deal of pain. Shoulder implant coatings are currently made of a variety of materials, including titanium, cobalt chrome, ceramics and pyrocarbons. Pyrocarbon is a very lightweight material that is preferable to traditional steel coatings which are heavy and bulky. Implants coated with titanium are lightweight and have a similar consistency to that of bone. Shoulder implants coated with advanced materials exhibit exceptional bone ingrowth qualities which gives the implant superb strength and the patient increased range of motion.

Knee: Recent advancements in titanium coating characteristics have allowed medical manufacturers to develop knee implants that exhibit superior bone ingrowth to traditional implants. It is the porosity of titanium that allows for bone ingrowth and thus makes it ideal for knee replacement implants. Improved bone ingrowth reduces recovery time and increases range of motion in knee replacement patients. Hydroxyapatite and titanium implant coatings have made joint implant surgery a more viable, less painful option for many people suffering from joint pain.

Since my Mom’s partial hip replacement surgery in late July she has reverted to her default position when she experiences tough times in various forms-”I want to die.” “You know what I want. I want to be dead,” is her mantra. In response, I have used words to encourage her to want to live. I offer her something to look forward to, such as returning to her day enrichment program. I tell her that God will call her when he’s ready for her to help all the people he has close and for now God wants her to help all the people who are alive. Indeed, when Mom is less depressed she only thinks about helping others. After seven days of our ping pong game of engagement, I realized I was hitting my head against the brick wall of resistance.

How was my resistance to hearing Mom’s mantra serving her or myself? The exchange only reinforced each of our positions, mentally, emotionally and physically. I was not meeting her where she was. We did not connect as well as usual.

For years as an organizational change consultant I worked with groups that were steeped in resistance to leadership change or technology change. Recognizing that resistance, naming it, spending time with it, exploring its benefits and costs and embracing it were essential to moving through it and forward to acceptance of the new leadership team and new technologies in the workplace.

Resistance is often more easily recognized when working with individuals than with groups, and is such a valuable place from which to get traction. In helping business coaching and personal coaching clients master the art of thriving in tough times, resistance appears early on in the coaching relationship.

After a week together with Mom, I was gob-smacked when I recognized I was operating from full on resistance. Here is what I did and the results.

  • Action-After she repeated her mantra the night of my recognition, I said, “Yes. That is how you feel. I have been responding to you from my perspective. I don’t have your perspective and don’t have your life experiences. I understand and accept as best I can from your perspective.”
  • Outcome-I could feel her upper body relaxing, her voice was a bit fuller and I sensed she was calmer. So was I. 

Tip
: Pay attention to when you find yourself working to persuade someone or influence someone rather than easily engaging in a conversation. Notice if your body is a bit tight from the waist up. Check in with yourself to discover if there is a pattern that resembles hitting your head against a brick wall. Experiencing any of these is a sign of resistance.

39 WEEKS PREGNANT, A LITTLE MUCUS, A FEW CONTRACTIONS & A LOT OF HIP PAIN?

hello to all my beautiful pregnant ladies!
today i am 39 weeks pregnant!
a few days ago at my doctors appointment i was told im 1.5 centimeters dilated (which i know is no big deal at all). since then, ive been getting irregular contractions… last night they were kinda hard (but still irregular). yesterday i had a little brownish mucus (tmi haha) when i wiped…. but i havnt had any since. i dont know whats going on! i want this kid OUT! the doctor said hes very very low and in position to come out. im also getting HORRIBLE hip pain…. oh my goshhhh. what do you ladies think? i just want some opinions! due date is august 24th…. do you think hes coming on time, early, or late? oh, and this is my 1st pregnancy! thanks everyoneee! im just so excited & anxious for him to be born :)
