Lower Back And Hip Pain, Important Information You Need To Know

Having lower back and hip pain now a days seems very common. At least four out of five people will have it at some point in their life. Some people tend to have more symptoms than others and some factors are controllable, while others are not. Educating yourself more about lower back and hip pain is the first step towards being pain free.

The Three Categories Of Lower Back And Hip Pain

The three categories of lower back and hip pain are determined depending on how long your pain lasts and how often it occures. If the pain lasts less than three months, it is referred to as acute. Most back pain is of this type. It is easy to deal with and there is no need for a doctors care. Get yourself a few days of bed rest, then back on your feet and work through the pain. Exercise will assist with working out the pain, as will analgesics.

Some people suffer from recurring acute pain. While it is bothersome and frustrating to have the pain return, if you treat it carefully as before, it should go away again, hopefully forever this time.

If you have lower back and hip pain last longer than three months, and you are in pain most of the time,then you have what is known as chronic back pain. It is advisable that you talk to your primary care physician, or try an alternative method, such as chiropractic care or acupuncture. Be sure to find a qualified professional, as you do not want to aggravate the problem!

Hip and lower back pain during pregnancy changes the rules. Pregnant women can have backaches off and on throughout their pregnancy, primarily due to the growing uterus and straining ligaments. But once the baby is delivered, the back pain should ease.

Likely Causes?

There can be many causes to lower back and hip pain. The most common cause being muscle strain, sciatica, an uneven pelvis or even one leg shorter than the other. You could have flat feet, or you even have very weak muscles that do not do an adequate job of protecting your spine. There are also other things that can cause lower back and hip pain.

Piriformis Syndrome: What Is It?

Piriformis syndrome is an inflammation in the piriformis muscle. This muscle is found very deep within the buttock, and it is the muscle that helps our hips to rotate. If there is tightness in the hamstring, the piriformis can become inflamed and sore, even to the outside back section of the buttock. It can swell and put pressure and inflammation on the sciatic nerve too, causing sciatica. Sitting can also aggravate this situation.

To relieve the lower back and hip pain and swelling, it is wise to use ice (wrapped in a towel for twenty minutes, several times a day). Taking an anti-inflammatory medicine will also help, if you do not mind taking a drug to ease matters. If touch is tolerable, massage the muscle deep into the tissues. Also strengthen and stretch out the hip area, using exercises targeting the hip, lower back and hamstring muscles.

Hip Pain And How To Address It

The hip area is a strictly defined region. In anatomy, the hip is the bony projection of the femur which is known as the greater trochanter, and the overlying muscle and fat. The hip joint, scientifically referred to as the acetabulofemoral joint, is the joint between the femur and acetabulum of the pelvis and its primary function is to support the weight of the body in both static (e.g. standing) and dynamic (e.g. walking or running) postures.
 
In my chiropractic practice I often see patients who tell me that they have pain in the hip. On examination they may have pain in the low back region, in the buttocks or in the sacroiliac joint. It would seem that hip pain can be difficult to describe and difficult to pin down to a specific problem.

One of the first things that I do in these cases is to ask the patient to have their low back x-rayed, This will include both hip joints. Other tests will be orthopedic, neurological and range of motion tests to see exactly which movements are both restricted and painful.

Let us look at some causes of hip pain.
 
1. Hip bursitis. This means inflammation of the bursa over the outside of the hip joint.
2. Osteoarthitis. This means wear and tear or degenerative joint disease.
3. Hip fracture. This means a broken bone.
4. Osteomyelitis. This means an infection in the bone itself.
5. Osteonecrosis. This means a restricted blood supply to the bone is causing the bone to break down and die.

Any of the above conditions require medical treatment and are not within the province of a chiropractor to deal with.

When do you need to call your doctor about your hip pain? If you are unsure of the cause of your symptoms, or if you do not know the specific treatment recommendations for your condition, you should seek medical attention. Treatment of hip pain must be directed at the specific cause of your problem. Some signs that you should be seen by a doctor include

* Inability to walk comfortably on the affected side
* Injury that causes deformity around the joint
* Hip pain that occurs at night or while resting
* Hip pain that persists beyond a few days
* Inability to bend the hip
* Swelling of the hip or the thigh area
* Signs of an infection, including fever, redness, warmth
* Any other unusual symptoms

Having drawn your attention to the need to seek medical treatment of the above conditions, let me assure you that other problems such as sacroiliac and low back stiffness and pain  frequently respond very well to chiropractic treatment. This includes pain that radiates into the buttocks and sometimes down a leg. This occurs when the sciatic nerve becomes irritated as a result of a subluxation or fixated joint.

For nearly 120 years chiropractors have successfully treated most of these conditions and their patients have been able to resume a full and normal life.

I have hip pain when I run, walk, and stand. What could this be? I run a marathon in less than 5 weeks!?

I am on week 12 of a 16 week marathon training schedule. Once the mileage started to increase I noticed some discomfort in my hips but it wouldn’t keep me from running. During week 11 I couldn’t even finish a 5 mile run. I rested for 4 days and today ran 3 miles and it felt ok during the run but the rest of the day it has hurt. It hurts on the outside of my right hip but is started to move to the front. It hurts to put pressure on it at all and it feels like growing pains in my whole leg while I lay and sit. I run the marathon on Oct. 17th. Any ideas what this could be and what I should do w/ my training?

If you are a beginner at running, then you shouldn’t increase milage until you know the time is right.

A 16 week marathon training schedule sounds like one for someone that is already experienced and has a body that has been running for quite some time.

If the amount of distance you run a week has increased by 10~20% within a couple days time then that could ultimately be the answer. Increasing your milage abruptly is a very simple way to cause injury. Running a marathon takes a lot of practice, and that includes at least half a year if you’re truly dedicated. I’ve had hip pain before, but I doubt it was from over doing my milage, but through playing golf and over-doing my form. Nonetheless, I know the pain you’re going through and it’s best to lay off it or get it treated as soon as possible. Keep it iced so that it could recover quicker and continue to go out for easy runs (as long as it doesn’t hurt you). If it doesn’t hurt you while you’re running then run what you think you are capable of, but don’t over do it. The main reason to keep running during an injury is to ensure that you don’t lose all the conditioning that you have already gained. If you cannot run, then attempt to go on a bicycle and ride around. If you plan to run 3 miles, then ride 7~10 miles.

When should I consider hip replacement/resurfacing?

I am 48 yo male. Until 3 years ago I was very active (running 50-80 miles/week, playing Ultimate disc), then I was diagnosed with arthritis in the right hip. Though I had no pain, the x-rays showed very advanced – almost no cartilage, serious flattening of the joint, and large bone spurs.

3 years later there is still almost no pain, but stiffness is making it difficult to get on a bicycle, tie my shoes, get in and out of cars… The general rule I have heard is avoid surgery until you can’t take the pain. My question is, is this still true? Isn’t it better to have it when still in better shape, rather than have the joint and muscles degenerate over several more years?

And, what are the prospects for resurfacing? The bone is solid, but misshapen. Does this rule out resurfacing?

I know this is not the best forum, but I live in China and am only in the US a few weeks a year, not enough time to investigate fully. Thanks.

To be honest, only an orthopedic surgeon would be able to tell you whether you are a candidate for resurfacing or not. It would be presumptuous of me (or anyone else) to tell you what your prospects are for resurfacing. This procedure has been in clinical trials in the US and the device has been recently approved by FDA. It is performed more frequently abroad – in Europe, for example and some Americans have traveled abroad to have their hips resurfaced. An example of a European OS who does resurfs: DeSmet of Belgium. In the recent past, the only way one could get the procedure in the US was to participate in a clinical trial. Now that the device has been approved by FDA, there will likely be an increasing # of US surgeons who do it. No matter what procedure you have, one of the important factors in making your choice of surgeon is whether the surgeon has done lots of hips on people like yourself.

There are still more surgeons that do THRs in the US. This may change. Only you can decide how long you can put up with the discomfort and increasing disability. I waited too long to have my hips replaced. They got so bad I couldn’t walk (needed a cane) and couldn’t travel, and basically my world got ever smaller. While I did fine after the surgeries, I do wish that I had opted to have them done a little sooner (nothing like hindsight!).

Do be aware that opinions on resurfacing vary and it is still a somewhat controversial procedure in the ortho world. However, many people have had one and done fine. By the same token, many more have had THR and done fine. What might cause groin pain in women?

I’m feeling pain, best described as if the elastic of my undies are extremely tight (but they arent, not even slightly tight). The pain is exactly where the elastic is and only one leg. It feels pinched & aches constantly, sitting, standing, walking, sleeping. It’s been going on for a few weeks. It’s tollerable, but concerning. I dont think I’ve strained anything, I dont think its related to my cycle, there is no swelling or bulging. Could it be a flare up of Sciatica? Back in May I had my 1st experience of Sciatica with extreme outter hip/buttock pain on the same side, but it was a completely different pain – radiating – sharp – I could barely walk w/o limping. I’ve searched “groin pain” and nothing mentions a similar pain to having a tight string choke your leg.
To clarify: When I refer to the elastic, it’s the elastic leg-hole of undies, not the wasteband elastic.

sounds like bursitis. That is the inflammation of the bursis which are on your outer hip and the pain radiates to groin too. I have that and when it flares up is acts like what you described as sciatica and then it will move to include the entire hip joint, which goes along your panty line, by the way. I sleep with a pillow between my knees and that helps alot, (I also eat ibuprofen like Skittles but thats another story). Ice pack on any of the painfull areas will help to ease the pain and inflammation. Look up bursitis on mayo clinic web site or webmd and there will be more info for you.
